Information for Cook County Residents

CountyCare: Cook County Health and Hospitals System's Medicaid program for uninsured adults 

The Cook County Health and Hospitals System (CCHHS) created a Medicaid program through the Affordable Care Act (ACA) called CountyCare. 

Who is Eligible for CountyCare?

To qualify for a CountyCare Medical Card, individuals must:

  • Live in Cook County

  • Be 19 - 64 years old

  • Have income at or below 133% of the Federal Poverty Level ($14,856 individual, $20,123 couple - annually)

  • Not be eligible for "State Plan" Medicaid (parent, pregnant, blind, or reciving disability income)

  • Not be eligible for Medicare

  • Be a legal immigrant for 5 years or more, or a U.S. citizen

  • Have a Social Security number, or have applied for one

CareLink

CareLink is a financial assistance program established for Cook County residents who are uninsured or underinsured and who receive medical care at John H Stroger, Jr. Hospital or any of its partner clinics. CareLink covers the cost of health care services provided at Cook County Health and Hospital Systems (CCHHS). CareLink allows CCHHS financial counselors to screen, help, apply, and approve patients for assistance programs faster than before through a streamlined process.

What does CareLink cover?

  • Visits to clinics

  • Laboratory visits

  • Hospital stays

  • Prescriptions

  • Urgent and emergency care

You qualify for CareLink if you...

  • Are a Cook County resident

  • Are uninsured or underinsured

  • Have an income below 600% of the Federal Poverty Line ($90,780 annually for a family of 4)

What are some documents I may need to apply for CareLink?

  • Proof of Cook County residency

  • Photo identification
